Male sexual health and genital aesthetics is one of the fastest-changing areas of modern medicine. In the past, men troubled by penile size or premature ejaculation were generally directed towards surgery requiring general anaesthesia and long recovery. Advances in medical aesthetics and biomaterial science now offer less invasive options. One expression of that change in Turkey is the «Damat Dolgusu» concept developed and trademarked by Op. Dr. Ahmet Semih Güleser. The name translates roughly as “the groom’s filler”.
Damat Dolgusu is a protocol that addresses both an increase in penile circumference and, through treatment of the glans, a supportive approach to premature ejaculation. The use of hyaluronic acid fillers for both penile girth enhancement and premature ejaculation has been evaluated in numerous clinical studies published in international peer-reviewed journals. This page sets out the details of the procedure, the filler technologies used, and evidence-based answers to the questions patients ask most often.
Why the Name, and How Did It Come About?
Penile size and girth can affect body image and self-confidence in many men. The condition described in the literature as small penis syndrome is a disturbance of perception that causes anxiety regardless of whether the penis is genuinely small, and most filler studies have assessed this patient group. This anxiety often becomes more pronounced in the period before marriage.
The registered term «Damat Dolgusu» describes not shaft girth enhancement alone but the combination of glans filler with shaft girth enhancement in a single protocol. The name reflects an approach in which the aesthetic concerns and worries about premature ejaculation of men preparing for marriage are addressed without the need for surgery. Because Damat Dolgusu is a registered trademark, treatment under that name is carried out only at authorised and supervised locations.
Does It Cause Erectile Problems? Is It Safe?
The idea of injecting a substance into the penis naturally raises the question “will my erection mechanism be harmed?” The reason the safety profile of this procedure is high is that the filler is placed in an anatomically superficial layer.
The structure that produces an erection is the spongy tissue inside the penis, the corpus cavernosum. In Damat Dolgusu, the target is neither inside nor beneath these vital structures; the filler is placed between the outermost skin layer and the loose subcutaneous tissue, away from the vascular and nerve network. The increase in volume therefore takes place in a superficial plane and the blood flow producing erection is not compressed.
Published clinical studies report that penile girth enhancement with hyaluronic acid is a safe and effective method with a low rate of side effects; in one multicentre randomised trial, filler-related side effects were reported at around 5–8% and were generally temporary. A large retrospective series of approximately 500 patients also showed that adverse effects were limited. As with any medical procedure, individual risk assessment is nevertheless essential.
Which Fillers Are Used? What Is the Hybrid Method?
The most critical factor determining success in non-surgical girth enhancement is the structure of the material injected. Hyaluronic acid (HA) occurs naturally in the human body and can hold a high volume of water. Systematic reviews and meta-analyses report that HA fillers give more favourable results than alternatives such as polylactic acid in terms of increase in diameter and patient satisfaction.
Monophasic HA fillers take the form of a single, smooth, homogeneous gel; because their cohesion is high, they do not separate when the tissue is stretched and they flex with it. This allows a natural feel without irregularity under the skin. Biphasic fillers, with their particulate structure, more strongly stimulate new collagen production beneath the skin.
In the Damat Dolgusu protocol these two are used together in a hybrid method: monophasic and biphasic fillers are combined in the shaft to provide smooth circumferential girth together with collagen support, while softer HA fillers are preferred for the more sensitive glans.
How Is It Performed? Cannula vs Multipuncture
How the material is delivered matters as much as its quality. The method we prefer is the blunt micro-cannula technique. A single entry point is made at the base of the penis and a flexible cannula with a non-sharp tip is directed into the subcutaneous plane. Because the tip of the cannula is blunt it does not cut the vessels and nerves within the tissue; it pushes them aside as it advances, and the filler is distributed homogeneously from that single opening using a fan technique. This markedly reduces the risks of bleeding, bruising and intravascular injection.
For comparison, the literature includes studies in which filler is applied with the fan technique or with a multipuncture technique. Because multipuncture involves entering at many points along the penis, the risk of bruising and nodularity may be higher. We therefore do not use it for the main volumising step; it is only rarely used as an adjunct to correct very fine asymmetries.
Does It Help with Premature Ejaculation?
One distinguishing feature of this protocol is that, alongside girth enhancement, it can offer a supportive effect on premature ejaculation. One common cause of premature ejaculation is excessive sensitivity of the nerve endings in the mucosa of the glans. Friction during intercourse strongly stimulates these nerves and can trigger the ejaculatory reflex early.
Soft HA filler placed beneath the glans creates a buffer layer between mechanical friction and the nerve endings, helping to reduce excessive sensitivity. A sensiometry assessment is recommended beforehand for objective measurement of sensitivity.
What does the evidence say? The effect of HA injection into the glans on premature ejaculation has been examined in randomised controlled studies. In a randomised, placebo (saline) controlled study, intravaginal ejaculatory latency time (IELT) increased approximately 2.6-fold after HA injection, compared with a 1.1-fold increase with placebo. The effect was generally reported to last 6–12 months, with a low rate of side effects. Our own observations are consistent with this, but individual results vary. Importantly, the aim of the procedure is not to create permanent loss of sensation but only to balance pathological hypersensitivity.
What Volume Is Used? What Result Can Be Expected?
In an organ with a surface as large as the penis, an adequate volume of filler is needed for a satisfying increase in girth; the low volumes sometimes seen on the market, such as 3–5 cc, generally cannot produce a noticeable result. At our clinic a combination of high-quality fillers totalling 15–20 cc is applied to the shaft, and 2–4 cc to the glans where premature ejaculation is also being addressed.

Steps of the Procedure
- Measurement and planning: Sensitivity is measured and an individual plan is made.
- Painless numbing: Discomfort during the procedure is minimised with topical creams and fine local anaesthesia.
- Cannula application: A single entry point is used and the filler is spread homogeneously in a fan pattern.
- Shaping: Gentle massage is applied so that the filler takes an even shape.
- Same-day discharge: The procedure is completed in about 15–20 minutes and the patient returns to daily life the same day.
Recovery and Aftercare
The fact that the procedure is non-surgical does not mean there is nothing to observe afterwards. For the best result:
- Bathing and heat: No water on the area for the first 24 hours. Avoid very hot showers, hammam and sauna for the next 2 weeks.
- Sexual abstinence: At least 2 weeks (avoiding intercourse and masturbation) so that the filler integrates fully with the tissue.
- Massage: Simple shaping massages as demonstrated by your doctor in the first days.
- Exercise: Avoid heavy gym training in the first week.
As reported in the literature, mild redness, temporary soreness or swelling may occur in the first days; these usually resolve on their own. An important advantage of this method is that it is largely reversible: if necessary, hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase. Because of the rare complications that can occur, it is essential that the procedure is performed by an experienced urologist under sterile conditions.

How much girth does it add? Does it add length?
The primary aim is to increase the circumference of the penis. In published randomised trials the mean increase was around 2–2.3 cm; at our clinic, with high volumes and a hybrid technique, increases of up to 3–4 cm are aimed for. This varies between individuals. Because the suspensory ligament is not cut, the procedure does not add measured length; however the added volume can make the penis look fuller.
Is it painful, and can it cause erectile problems?
The procedure is generally well tolerated because numbing creams and local anaesthesia are used. The filler is injected into the superficial subcutaneous tissue outside the erectile chambers (corpus cavernosum), so it does not damage those structures and is not expected to impair erectile function.
Is it permanent? How long does it last?
Hyaluronic acid is biocompatible and is naturally absorbed over time, so it is not permanent. The literature reports that the effect generally lasts 12–24 months. High-quality fillers may stimulate some collagen production while they remain under the skin, so a partial volume may persist for a while after absorption. Durability varies with the individual and the product used.
Does it cause loss of sensation? Will my partner notice?
Permanent loss of sensation is not expected. When applied to the glans it helps to balance the excessive sensitivity that contributes to premature ejaculation, and studies have reported a marked increase in ejaculatory latency. Smooth gel fillers applied with a micro-cannula technique aim for a natural tissue feel.
When can I return to sexual activity and normal life?
The procedure takes about 15–20 minutes and normal daily life can be resumed afterwards. To protect the result, no bathing is advised for the first 24 hours and sexual abstinence for the first 2 weeks. Sexual activity can resume after 14 days.
How much does it cost?

Can it be reversed?
Yes. Hyaluronic acid based fillers are absorbed naturally over time and can also be largely dissolved with the enzyme hyaluronidase if needed. This is one of the most important safety advantages of the method and allows correction of an unwanted result.
